I acknowledge that during my employment with ServiceMaster Restoration & Cleaning, I may have access to confidential information about the clients that must NOT be disclosed to any other person(s) / business(s) unless in pursuit of my duties or without specific permission given by ServiceMaster Restoration & Cleaning.

I agree that this confidentiality applies during my working relationship with ServiceMaster Restoration & Cleaning and after the relationship ceases.

I acknowledge that confidential information includes all information relating to the client, their premises and personal property.

I will not pursue employment with any ServiceMaster Restoration & Cleaning clients on a personal/business basis during or after my employment with ServiceMaster Restoration & Cleaning.

If I am in doubt as to what information may be disclosed, I will check with the ServiceMaster Restoration & Cleaning Production Manager for clarification.

• I authorize investigation of all statements contained in this application and supporting documents. I authorize the company to secure information about my experience from former employers, educational institutions, government agencies, and any references I have provided, and for those parties to provide information concerning my experience, and I hereby release all parties from liability arising from such investigations. I specifically authorize investigation of my DMV record, criminal record, and consumer credit history.
• If I am offered employment, I will, as a condition of employment, be required to submit proof of my identity and legal right to work in the US.
• If I am offered employment, I will, as a condition of employment, furnish proof that I am over 18 years of age.
• If I am employed, any false statement, misrepresentation, or omission of facts on this application or any supporting documents, regardless of when discovered to be false, will result in my immediate termination.
• I will be required to possess a current valid driver's license and be insurable if my job requires me to drive in the course of my work.
• I understand that anything I see or hear on the job is confidential and I may not share any information about my work.
• I understand that any offer of employment may be contingent upon my passing a job-related physical examination performed by a healthcare provider selected by the company.
• I understand that the company reserves the right to require me to submit to drug and alcohol screens and/or testing at any time during my employment, to the extent permitted by law.
• I agree that, if I am offered a position, I will be required to conform to the rules and regulations of the company.
• I agree that, if I am offered a position, it will be offered on condition that my employment shall be at will and for no definite period, and that my employment may be terminated at any time with or without prior notice.
• I understand that, as a condition of employment, I may be required to enter into an employment agreement which may include an agreement to arbitrate all disputes arising out of my employment or the termination of my employment.
• I am responsible for any and all keys, fobs, codes, or any other means of accessing the buildings I clean. I will be held liable for any lost or stolen keys and cost of re-keying or lock changes, pass code changes, etc.
• I am responsible for any and all ServiceMaster Restoration & Cleaning owned equipment, tools, meters, etc. in my care. I may be held liable for damage due to negligence and am required to return all such items upon termination of employment.
• Pay days are bi-weekly on Friday, unless those days fall on an office closure or Holiday, in which case payment is made the following business day via direct deposit.
• I am responsible for my time card and punching in and out of work. I must inform the manager of any corrections needed.
• Vacation requests must be submitted to the manager before the requested date or they will not be approved.
